Between July 2009 and March 2011, Northamptonshire successfully hosted one of only 16 national integrated care pilots. The pathways developed under its remit are now part of core business accross the Northamptonshire health economy

Nene Commissioning is the lead organisation for this work which involves health, social care and third sector partners across the county.

The Northamptonshire Integrated Care Partnership built on existing joint working to develop new models of long-term condition management to help patients remain independent for longer and have more choice in their end of life care. It creates personalised care plans for high-risk individuals and aims to reduce admissions to hospital.

Over the past two years the NICP has achieved and introduced significant service change across primary, secondary, social and 3rd sector services including a new community based service for patients at the end of their life and the community elderly care service to assess and support elderly, frail patients to remain independent.
