Northamptonshire is hosting one of only 16 national integrated care pilots.  Nene Commissioning is the lead organisation for this work which involves health, social care and third sector partners across the county. 

The Northamptonshire Integrated Care Partnership is building on existing joint working to develop new models of long-term condition management to help patients remain independent for longer and have more choice in their end of life care. It will create personalised care plans for high-risk individuals and aims to reduce admissions to hospital.
