This is a case management system for looking after people at high risk of hospital admission, which allows them to stay in their own homes.

Care Homes
 
Improving the provision of general medical services in care homes and supporting care homes with training and equipment has been one of our major schemes. We have also implemented a scheme for providing pharmacy advice into care homes which is improving quality of care and safety for these patients.

ScriptSwitch is an award winning prescribing tool which assists GPs in decision making at the point of prescribing which we have been piloting with practices in Nene Commissioning.

Community Intravenous Therapy
 
We have a scheme of administering IV therapy in the community for patients with cellulitis.  This is being reviewed to ensure we maximise the number of patients who can receive this treatment at home.

Two schemes are developing where primary care staff are working in partnership with clinical teams in our acute hospitals to help direct appropriate patients to community services and so reduce the number of admissions.

We have been involved in a partnership with NHS Northamptonshire and Northamptonshire County Council Social Services which is developing a new Intermediate Care Strategy.

Leg Ulcer Community Service
 
We are reviewing leg ulcer clinics in Northampton and Daventry and aim to accommodate increased demand and initiate new clinics in Wellingborough, Kettering and Corby.
